Under the umbrella of the non-profit and non-commercial Siloah Foundation, the Siloah network consisting of hospital, long-term care, Spitex as well as operational and IT functions with around 1,000 employees forms the competence center for integrated care in old age. The Siloah network is currently active at five locations in the canton of Bern and sees itself, among other things, as a solution provider for long-term and Spitex organizations in operational functions.
At the location in Gümligen, we are looking for a social counselor from January 1, 2026, or by arrangement
Your tasks
- You are the central contact and counseling person for patients and their environment during the hospital stay
- You take over discharge planning on behalf of the medical staff and advise patients and their relatives on inpatient follow-up solutions (nursing and retirement homes) and take over the planning and mediation of follow-up care or relief services (Spitex, meal service, household help, day care, care service, etc.) and transport services
- You advise on social insurances (especially supplementary benefits, helplessness compensation)
- You work interdisciplinarily with all involved parties (doctors, nursing, therapists, external social services, hospitals, homes, KESB, etc.) and ensure coordination and networking
- You support patients on request in filling out a self-report for a guardianship
- You document cases in writing in the "CGM Clinical" system
Your profile
- You have a completed degree in BSc Social Work
- You have knowledge of Swiss social insurance law
- You already have experience dealing with elderly people
- Good communication and organizational skills characterize you
- You enjoy teamwork as well as interdisciplinary collaboration
- Good computer skills complete your profile
- You have completed the practical training course in social work or are willing to attend it later
- Willingness to work on Fridays
