Nursing Specialist HF (DN II/AKP/GKP) 40-70%

What awaits you:
As a nursing specialist, you take on the professionally demanding care, nursing, and preventive support of our clients in their familiar environment.
You conduct needs assessments (RAI-HC), coordinate with doctors and hospitals, and advise relatives.
You document the nursing process professionally and, if desired, contribute to the training and supervision of trainees.

What you bring with you:

• Diploma in Nursing HF or equivalent qualification (DN II, AKP, GKP)
• Professional experience in outpatient care is an advantage
• Enjoyment of personal responsibility and independent work
• Motivation to help shape new things
• Driver's license category B

Your team:
You work in an interdisciplinary, self-organized team with 7–12 colleagues that:

• plans, organizes, and decides independently
• flexibly arranges working hours (e.g., focus on evening or weekend shifts)
• operates without a formal manager – instead with clearly distributed roles
• selects new team members independently
• relies on personal responsibility, trust, and collaboration
