Your tasks

• Provision of follow-up outpatient assistance such as outpatient assisted living, self-help groups, nursing services, nursing aids
• Placement in inpatient services such as integration assistance facilities or nursing homes
• Application for benefits from nursing care funds, health insurance companies, and other cost carriers
• Networking
• Reintegration into society of the patients treated here (m/f/d) as well as the mediation and initiation of rehabilitation measures
• Organization and coordination of long-term therapy places in coordination with clinics, cost carriers, and patients (m/f/d)
• Information and advice on social law issues regarding health insurance, nursing care insurance, severe disability law, and similar areas
• Initiation of guardianship procedures at the district court and advice on the topic of power of attorney
• Assistance in securing financial and material livelihood
• Participation in ward rounds and multiprofessional team meetings
• Independent conduct of discussion groups
• Discharge management

Your profile

• Completed studies in social work/social pedagogy (Diploma or Bachelor)
• Several years of professional experience in psychosocial work, especially in dealing with people with mental illnesses and addiction problems
• Motivation and commitment to working with people with addiction
• Knowledge of relevant social laws
• You act motivated and responsibly and enjoy working independently in a multiprofessional team
• Communication skills and team spirit
• Willingness to cooperate with other professional groups
