Your tasks
- Provision of follow-up outpatient assistance such as outpatient assisted living, self-help groups, nursing services, nursing aids
- Placement in inpatient services such as integration assistance facilities or nursing homes
- Application for benefits from nursing care funds, health insurance companies, and other cost carriers
- Networking
- Reintegration into society of the patients treated here (m/f/d) as well as the mediation and initiation of rehabilitation measures
- Organization and coordination of long-term therapy places in coordination with clinics, cost carriers, and patients (m/f/d)
- Information and advice on social law issues regarding health insurance, nursing care insurance, severe disability law, and similar areas
- Initiation of guardianship procedures at the district court and advice on the topic of power of attorney
- Assistance in securing financial and material livelihood
- Participation in ward rounds and multiprofessional team meetings
- Independent conduct of discussion groups
- Discharge management
Your profile
- Completed studies in social work/social pedagogy (Diploma or Bachelor)
- Several years of professional experience in psychosocial work, especially in dealing with people with mental illnesses and addiction problems
- Motivation and commitment to working with people with addiction
- Knowledge of relevant social laws
- You act motivated and responsibly and enjoy working independently in a multiprofessional team
- Communication skills and team spirit
- Willingness to cooperate with other professional groups
