Tasks
- Begleitung und Betreuung der Bewohnenden im Bereich Palliative Care.
- Kommunikation mit Bewohnenden, Angehörigen und dem Team.
- Führung der Pflegedokumentation sowie Erstellung der Pflegeplanung.
Skills
- Abgeschlossene Ausbildung als Dipl. Pflegefachperson HF oder Äquivalenz.
- Gute Deutschkenntnisse in Wort und Schrift (mindestens Level B2).
- Hohe Sozial-, Methoden- und Fachkompetenz.

The Dielsdorf Health Center is a modern geriatric competence center with around 500 employees from 35 nations. Our services range from advisory services for elderly people and their relatives to general and specialized long-term care as well as home care and support. With over 200 beds, the inpatient general long-term care offering at the Dielsdorf location is complemented by specialized treatment pathways for rehabilitative acute and transitional care, palliative care, psychiatric geriatrics, and the focus on dementia.
- Accompanying and caring for residents with a focus on palliative care
- Communication with residents, relatives, and the team
- Maintaining nursing documentation and creating care plans
- Collaboration in the interdisciplinary team (nursing, medical service, therapists, as well as counseling and social services)
- Assuming daily responsibility
- Completed training as a Dipl. Nursing Specialist HF or an equivalent with SRK recognition
- Further training in the palliative area or willingness to complete such training
- Good German language skills in spoken and written form (at least level B2)
- High social, methodological, and professional competence
- Up-to-date IT skills
- Independent and responsible working style
