Job Description

• Ensure communication with the VIVA member client through the telephone hotline: 
  • Perform medical triage if there is a medical problem
  • Make appointments with family doctors, specialists, and/or therapists
  • Answer their questions.
• Prepare client files by updating them according to their care pathway
• Create individual health plans linked to the medical file and the specificities of member clients in a comprehensive approach, focused on prevention and management of complex cases
• Meet clients to personalize, adapt, and communicate their health plan
• Ensure orientation, coordination, and follow-up of member client care in collaboration with family doctors and the Care Network

Qualifications

• Nursing diploma with experience in various hospital fields
• Bilingual French - German
• Good clinical reasoning
• Good clinical knowledge
• Knowledge of common computer tools
• Professional awareness
• Good communication
• Interpersonal skills, ability to work in a team
• Autonomy, sense of organization and coordination
• Sense of priorities and good stress management
