We are looking for a certified nursing specialist HF/FH Palliative Care for 30% employment immediately or by arrangement.
Certified Nursing Specialist HF/FH Palliative Care
Your tasks
As a nursing specialist, you take responsibility for the entire care process of our clients. Your focus is on efficient, evidence-based care with an emphasis on primary nursing. You coordinate with institutions, doctors, and insurers to ensure optimal support. Your tasks include needs assessment, counseling, and support of clients, always keeping individual needs in mind. You work interprofessionally, especially in complex and unstable situations in the home environment.

Your profile
- Certified nursing specialist HF or equivalent qualification
- Specialist training in palliative care / support in obtaining B2 if necessary
- Evening and weekend shifts and on-call night duty with medical background service
- High professional and social competence
- Resource-oriented attitude in nursing
- Teamwork, independence, and reliability
- Knowledge of interRAI HC and Perigon tools is an advantage
- Driver's license category B and own car
