Internal Social Service Employee 40%
Your area of responsibility
- Recording the social support needs within the framework of inpatient treatment in the hospital as well as in our Alberti nursing home
- Participation in interdisciplinary meetings on patient situations
- Triage, discharge planning, and organization of concrete assistance (housing, residential, elderly and nursing institutions, etc.)
- Close cooperation with the Health Counseling Center of the Municipality of Davos
- Coordination and exchange with external agencies
Your profile
- Certified nursing professional HF or equivalent qualification
- Expertise in social insurance law or willingness to acquire it
- Solution-oriented, empathetic as well as decisive and forward-looking personality
- Passion for organizing and coordinating in a holistic approach for patients
- Very good German skills in spoken and written form are required, any additional language is an advantage
- Good IT user skills
