Certified Nursing Specialist HF/FH

As a Nursing Specialist HF/FH and Case Manager 

• you independently coordinate and organize holistic care
• you advise and support elderly and young people and their environment - often over a longer period
• you clarify and check the needs with interRAI HC and create the care plan with NANDA diagnoses
• you plan treatment care in consultation with doctors and therapists 
  you ensure a high quality of care and service
