Your tasks
- Planning and organizing the return to the social "reception area" (school, daycare, training, etc.) at the end of the (partial) inpatient treatment and continued care in the outpatient setting
- Accompanying patients on site
- Advising all external reference persons involved in the process
- Close cooperation with the (partial) inpatient treatment teams and the school for the sick
- Independent implementation of group offers
- Occasionally working in the nursing and educational teams of the wards
- Constant exchange with schools/school social workers in region 10
Your profile
- Completed studies in social work with state recognition
- Professional experience in clinical social work
- A high degree of flexibility, resilience, commitment, and ability to work in a team
- Structured and independent work
- Empathy, organizational skills, communication, and assertiveness
- Willingness to cooperate with the different stakeholder groups
