A colleague is expecting a child, and therefore we are looking for a humorous, warm, and competent substitute who will lend a hand from April 1, 2026, to December 31, 2026. And who knows: Maybe a reduced workload will be possible from January 2027.
This is where you get involved
- in clarifying the social situation of patients as well as organizing outpatient follow-up care and developing sustainable solutions to avoid rehospitalization for social reasons
- in professional advice and psychosocial support for patients, relatives, and those affected during the hospital stay
- in organizing and leading interdisciplinary roundtable discussions
- in mediating nursing homes or other social therapeutic facilities as well as advising on financing and organizing the transition
- in organizing rehabilitation and cures including obtaining cost approvals
- in interdisciplinary collaboration with internal and external professionals, institutions, authorities, and counseling centers in health and social services
- in clarifications and initiation of official measures
- in advising on social insurance issues and assisting in accessing benefits and financial resources
- in participating in interdisciplinary meetings
You
- have a university of applied sciences degree in social work
- already have professional experience (advantageous)
- have knowledge in the field of health and social policy as well as social insurance issues
- have strong communication and teamwork skills, negotiation skills, and assertiveness
- possess empathy and sensitivity in dealing with patients and their relatives
- have the ability to act calmly and solution-oriented in stressful situations
