About the job

Your main missions will be the following:

• Take care of people in vulnerable situations;
• At the request of the CHUV hospitalization services, ensure the transitional follow-up of people in vulnerable situations upon hospital discharge;
• Support the nursing staff in complex care situations;
• Collaborate with other units in the sector in an interprofessional manner;
• Ensure a role as transition case manager;
• Actively participate in the development of the EmvS unit (vulnerability mobile team) and its operation.

Your assets:

• Bachelor's degree in nursing or equivalent qualification;
• Minimum 2 years of nursing practice in the community setting;
• Mastery of French orally and in writing, knowledge of English and/or another language is desirable;
• Adaptability, ability to manage complex, conflictual and emergency situations;
• Sense of collaboration with multidisciplinary teams and autonomy;
• Ability to decide and to be a driving force for proposals.
