Integrated into a medical environment, you provide social and administrative support to patients attending partner medical practices by facilitating access to social aids and benefits.
Your intervention contributes to efficient coordination between health actors and social services while also aiming to support medical teams by addressing social issues related to patients' situations so that medical teams can focus on medical follow-up.
Your main activities consist of:
- Assessing the social, economic, and family needs of patients.
- Informing, advising, and directing patients to appropriate social services (social insurance, allowances, housing aid, psychological support, etc.).
- Supporting patients in their administrative procedures (file preparation, contacts with relevant institutions, follow-up of requests).
- Working closely with doctors and paramedical staff in a coordinated and interdisciplinary approach.
- Documenting interventions and follow-ups in compliance with legal requirements regarding confidentiality and data protection.
- Collaborating in the evaluation carried out within the framework of the pilot project.
Your profile:
- Bachelor's degree in social work (social assistant orientation) or equivalent training.
- Professional experience in social assistance, ideally in a medical or paramedical environment, is an asset.
- Good knowledge of the Valais social network and social insurance.
- Autonomy, rigor, organizational skills, and methodology in social and administrative work.
- Ability to work in a developing environment and strong interest in innovative practices and interdisciplinary work.
- Excellent listening, analysis, and advisory skills.

The opportunity to actively contribute to the development and evaluation of a new position within the framework of an innovative pilot project.
Working conditions according to the collective labor agreement for long-term care, including notably a 13th salary, regular, transparent salary increases adapted to responsibilities, 25 days of vacation per year (30 days from age 40).
The possibility to choose between two pension plans and to adjust your contribution annually.
