For the Senevita Casa Ticino Luganese Region location, we are looking from April or by agreement for the following position
Qualified Nurse with Case Management 60%-100% (m/f) Luganese
Tasks
- Conduct and record RAI-HC needs assessments with clients and families
- Ensure the nursing process
- Prepare care documentation and set up planning
- Perform basic care services and examinations and treatments for clients
- Support the medical management in the introduction and training of new employees
- Support the medical management in ensuring the flow of information with external partners (doctors, health insurance companies, hospitals, …).
- Manage counseling, support, and complex care processes
- Practice and develop high-quality case management in the operational context aimed at strengthening the abilities and action potential of users
Requirements
- Nursing diploma (Bachelor SUPSI, SSSCI diploma or recognized diploma if obtained abroad)
- Continuing education InterRAI-HC with proven experience
- Inclination for patient-oriented quality care, interdisciplinary collaboration within the team, and professional and discreet transparent communication.
- Professional experience in home care
- Reliability, flexibility, organization, precision, punctuality, seriousness
- Car owner
