Nurse in Home Hospitalization (m/f/x)

To complement the Home Hospitalization service, we are looking for a nurse.

Job Description

• Coordinate different types of home hospitalization care pathways in support of the multidisciplinary team (doctors, pharmacist, physiotherapist, home care organization).
• Ensure the administrative follow-up of care management.
• Monitor the patient’s clinical condition, provide care in the home setting.
• Inform and educate the patient and their family.
• Be involved in training new staff and developing skills.
• Participate in the development of home hospitalization activities and the integrated care network of the Arc Network.

Qualifications

• Diploma in nursing, Red Cross recognition if foreign diploma.
• More than 2 years of experience in a service, comfortable performing technical care, willingness to develop clinical observation skills.
• Skills in using digital tools.
• Ability to work in a team and conduct home visits independently.
• Interpersonal skills, ability to establish a trusting relationship with the client.
• Possession of a driving license and vehicle.
