To complement the Home Hospitalization service, we are looking for a nurse.
Job Description
- Coordinate different types of home hospitalization care pathways in support of the multidisciplinary team (doctors, pharmacist, physiotherapist, home care organization).
- Ensure the administrative follow-up of care management.
- Monitor the patient’s clinical condition, provide care in the home setting.
- Inform and educate the patient and their family.
- Be involved in training new staff and developing skills.
- Participate in the development of home hospitalization activities and the integrated care network of the Arc Network.
Qualifications
- Diploma in nursing, Red Cross recognition if foreign diploma.
- More than 2 years of experience in a service, comfortable performing technical care, willingness to develop clinical observation skills.
- Skills in using digital tools.
- Ability to work in a team and conduct home visits independently.
- Interpersonal skills, ability to establish a trusting relationship with the client.
- Possession of a driving license and vehicle.
