The public Spitex Wald Fischenthal is responsible for the outpatient healthcare of the residents of Wald and Fischenthal. The Spitex staff competently and empathetically care for their clients in their own homes, provide psychiatric support, or assist them in daily life. Spitex Wald Fischenthal is part of the non-profit Stiftung Drei Tannen, which, in addition to Spitex, operates the newly built Rosenthal nursing center and offers senior apartments with needs-based services.

Your tasks:

• Individual care & support: You accompany our clients with heart and expertise in their daily lives.
• Case management & responsibility: You manage the care and support process and take on case management.
• Medical-technical measures: You apply your expertise purposefully to ensure the best possible care.
• Flexibility in deployment: Evening and weekend shifts are part of your varied workday.
• Interdisciplinary collaboration: You work closely with colleagues within Stiftung Drei Tannen, external partners, and relatives to ensure holistic care.
• Innovation: You understand change as part of life. You approach new requirements openly, think innovatively, and actively shape further developments.

What you bring:

• Diploma as a nursing specialist HF/FH
• Experience with RAI HC and digital work tools
• A communicative, team-oriented personality with good German skills
• Driver's license category B
• Initiative, self-responsibility, and flexibility – you take action and find solutions
