For our Team C located in Tafers, we are looking for a
Certified Nursing Specialist HF/FH 60%-80% to start immediately or by arrangement.
What awaits you
In the role of nursing specialist, you take over case management for clients. You are responsible for professional, client-centered, continuous, and coordinated
care and support as well as for planning and managing the entire care process.
This also includes conducting reassessments with the responsible clients using the RAI HC instrument to assess and register care needs with health insurance companies.
What you bring
You have completed training as a certified nursing specialist HF/FH and have a professional work attitude. You are professionally competent, resilient, and flexible and enjoy working independently. Your open and communicative personality significantly contributes to good teamwork as well as interdisciplinary exchange with our partners in healthcare. Working with PC and tablet is part of your daily routine. You have your own car available for assignments with our clients.
