YOUR TASKS
- Independent counseling and support of people with various types of diabetes, both in outpatient and inpatient settings.
- Close interdisciplinary collaboration with all medical professionals involved in care, both within the hospital and externally, to ensure holistic care.
- Learning and applying all technical aids in diabetology to ensure optimal care.
- Instruction and support of patients in the use of continuous glucose monitoring (CGM) and/or insulin pumps.
- Handling electronic data processing (KISS/Diabass, etc.).
- Supporting the medical consultation through professional and patient-oriented preparation and follow-up.
- Preparation and implementation of further training, education, and courses to inform and train both patients and professionals.
YOUR PROFILE
- Completed training as a certified nurse HF/FH, DN II, or AKP (with SRK recognition for foreign diplomas).
- Already completed further training in diabetes counseling (CAS/DAS/Höfa) or willingness to complete the Higher Professional Examination in Diabetes Counseling.
- Desirable: Knowledge in the area of CGM systems and insulin pumps.
- Committed, independent, and proactive personality with a structured, precise, and autonomous working style.
- Enjoyment of interprofessional collaboration.
- High flexibility, strong patient orientation, and excellent communication skills.
