Your mission will consist of ensuring the complete and autonomous follow-up of the assigned situations in reference, from evaluation to case closure, including collaboration with the relevant health partners and health insurance funds.
You know how to carry out and monitor evaluations on the InterRAI system as well as client intervention plans.
You are able to collaborate with all multidisciplinary teams and bring your knowledge with pedagogy and seriousness. You also know how to listen to colleagues and find constructive solutions combining quality care and group adherence.
Training and experience requirements
- Training and experience as a nurse
- Experience in the field of home help and care would be an asset
- Experience using InterRAI would be an asset
- Flexibility regarding workload variations related to the field of home help and care.
