Your Tasks
- Conducting and documenting structured nursing conversations (e.g., about medication adherence, daily structuring, psychoeducation)
- Crisis interventions in cooperation with medical and psychotherapeutic team members
- Participation in multiprofessional case discussions and treatment planning
- Support in outreach care (according to § 118 SGB V) in special needs situations
- Contributing to the further development of outpatient treatment processes
- Telephone contact with medical practices, clinics, and patients
- Planning and coordinating appointments for our outpatient team
- If necessary, performing ECG recordings, blood draws, or depot administrations
Your Profile
- Completed training as a health and nursing professional (m/f/d) or nursing specialist (m/f/d)
- Additional qualification in psychiatric nursing (specialist training)
- Solid knowledge in relationship building with complex traumatized individuals
- Teamwork skills, structured working style, and interest in interdisciplinary collaboration
- Willingness to independently and planned nursing intervention in the outpatient setting
- Knowledge of IT-supported documentation
