About the job
- You are part of our interdisciplinary treatment team and work closely with our specialists from the fields of medicine, psychology, nursing, and therapies, supporting patients in the outpatient and day clinic settings in Uznach and/or Rapperswil.
- After an orientation tailored to your needs, you accompany patients in the form of social counseling, job coaching, and case management with the goal of social and professional reintegration into life and work environments.
- In your daily work, you assess the current social situation of the patients, define social issues, and develop solutions in collaboration with those affected and our multiprofessional team.
- Your tasks include maintaining relationships with social insurance agencies, authorities, employers, and the relatives of the patients.
- At our location in Uznach/Rapperswil, you participate in reports, supervision, and further training and engage with our team in public relations.
- You have a degree in social work at the university of applied sciences level.
- Ideally, you bring professional experience from the psychiatric or medical field.
- You have good knowledge of the Swiss social system and are familiar with social counseling.
- Your interest lies in interdisciplinary treatment oriented to the needs of patients and their environment.
- You demonstrate social work commitment, empathy, and communication skills in dealing with mentally ill people and their relatives.
- Action and design scope are important to you, and you appreciate the possibility to organize your workday flexibly.
- You like to work independently, are resilient, and a team player.
