With nearly 1,500 employees, the Department of Rehabilitation and Geriatrics is a medical center focused on the elderly and is composed of 6 services spread over 5 sites: Trois-Chêne, Loëx, Bellerive, Beau-Séjour, and Joli-Mont. It manages acute situations, notably through the emergency department of the Trois-Chêne hospital, age-related specific problems, medical rehabilitation, and palliative care.
The Community Geriatrics Unit (UGC) cares for patients in complex situations by visiting them at their place of residence. To ensure this geriatric follow-up at home, the UGC team, multidisciplinary (doctors, nurses, ASSC, and administrative staff), collaborates with the Geneva home care network and cares for people aged 65 and over living in the canton of Geneva. Job description
As a nurse within the Community Geriatrics Unit (UGC), you assess clinical situations as part of intervention requests and ensure the appropriate triage and orientation of these. You guarantee the follow-up of patients' care pathways and coordinate the various services necessary for their care. You will also be required to travel to patients' homes to conduct nursing consultations; this activity will be developed in 2026.
You work within an interprofessional team and actively contribute to strengthening coordination among different professionals, especially in complex situations.
You also participate in the supervision and training of trainees as well as new employees.
Finally, you contribute to the development and continuous improvement of care quality by conducting professional reflection based on the analysis of nursing practices.
Qualifications
You hold a Bachelor of Science in Nursing (HES) or an equivalent nursing diploma recognized by the Swiss Red Cross.
Experience and knowledge of the Department of Rehabilitation and Geriatrics are a real asset. You also have a very good knowledge of the care network and master the geriatric assessment as well as the various scales and are able to perform clinical evaluations independently.
Furthermore, you possess great rigor as well as a keen sense of organization and priority management. You demonstrate excellent interpersonal skills and have a strong interest in the care of elderly patients.
Thanks to your team spirit and communication skills, you work interprofessionally to unite all partners around a common vision to develop care projects aimed at optimal care for patients and their relatives.
Comfort with computer tools is necessary to successfully carry out this mission, as well as a very good knowledge of the French language.
