The Foundation for Senior Housing of the City of Zurich (SAW) operates 36 settlements with more than 2000 affordable apartments for people aged 60 and over within the city area. It supports its tenants with everyday, social, and nursing services, thereby enabling them to enjoy a high quality of life into old age. Its diverse socio-cultural offerings support the development of neighborhood contacts and an active community life.

For the Spitex area, we are looking for a responsible certified nursing specialist HF with case management to start immediately or by arrangement.

Certified Nursing Specialist HF/FH with Case Management, 80%

Tasks

• You ensure qualified and resource-oriented care and support for our clients. For this, you work with various internal and external service providers.
• You take responsibility for the entire care process, including needs assessment using RAI-HC.
• You supervise and advise employees and trainees.
• You actively participate in social tasks for the foundation. This includes, for example, leading regularly held consultation hours in our settlements.

Profile

• Completed tertiary-level education such as Certified Nursing Specialist FH/HF or equivalent training
• Several years of professional experience in a comparable role within a Spitex organization
• Enjoyment and interest in working with elderly people
• Independent and responsible working style
• RAI-HC knowledge as well as proficiency in MS Office
• Driver's license category B
