- Case management with planning, control, and evaluation of the nursing process
- Resource- and goal-oriented care and support
- Consultation and support of relatives
- Interdisciplinary collaboration with internal specialist departments as well as external specialist offices, referrers, and doctors

- You are a certified nursing specialist HF
- You have empathy and heart for the care of elderly people in the home environment

- Good communication skills with an interest in coaching employees
