The position is attached to the Operations Management, which has a triple mission: to provide support services for patient care (reception, coordination of patient flows, patient transport), to integrate and ensure social follow-up with HUG patients and their relatives, and to support clinical activity in logistical and organizational aspects. Its organization is divided into 4 entities: patient flow management, reception and telephone switchboard, on-site patient transport, and the social service.

Job Description

As a social worker within HUG, you work with patients in the geriatrics and rehabilitation department using a comprehensive and multidisciplinary approach. You ensure the social follow-up of hospitalized persons, particularly in rehabilitation units, palliative care, and LSM beds (beds for patients awaiting placement).

Your role is to assess the social needs of patients and their relatives, identify social difficulties, and propose appropriate solutions. You handle administrative and financial requests and support patients in their efforts to maintain home care, rehabilitation, or transition to another facility (nursing homes, care structures). You work closely with the internal multidisciplinary team as well as external partners.

Your work is based on individual interviews, family meetings, and multidisciplinary conferences to establish a trusting relationship with the patient, understand their situation, and develop concrete and personalized solutions. You are also responsible for the rigorous transmission of patients' social needs and projects to the care team and external partners.

Finally, you actively participate in professional monitoring on social and legislative topics, particularly social insurance, and in the prevention of social problems. You are involved in the continuous improvement of practices within the service.

Qualifications

You hold a Bachelor of Science in Social Work or an equivalent recognized qualification.

You have in-depth knowledge of the social network in the Geneva area and social insurance.

Experience in social work in a hospital setting and/or geriatrics is an asset.

You are a person with excellent ability to constructively defend social expertise in a demanding and emotionally challenging interdisciplinary environment.

You have a sense of organization, the ability to manage priorities, and to work independently. You have excellent writing skills and proficiency in computer tools.

Additional training as a practitioner trainer and in social insurance are assets.
