Certified Nurse with Case Management for Inhouse Home Care 50-80%
Your Tasks
- You are responsible for the care and support of our complex cases
- You bear responsibility for nursing diagnostics
- You carry out assignments in basic and treatment care with our clients
- You advise and support our clients and their relatives
- You conduct needs assessments with RAI-HC at the client’s location
- You create nursing documentation and individual care planning
- You support the management in expanding our internal home care services
What You Bring
- Training as a certified nurse
- Several years of professional experience in long-term care or home care is an advantage
- You already have further training qualifications in the RAI-HC area or are willing to pursue further training soon
- You are resilient, flexible, and used to working independently
