The Dielsdorf Health Center is a specialized care institution with around 430 employees in the Zurich lowlands and ensures care at nursing places for the 22 political municipalities of the Dielsdorf district with approx. 90,000 inhabitants. With over 200 beds, the inpatient offer of general long-term care is complemented by specialized treatment pathways for rehabilitative acute and transitional care, palliative care, psychiatric geriatrics, and the focus on dementia.
The Regional Spitex forms an important part of the care offer. It provides outpatient services for the five contract municipalities. In the future, it is planned to establish a night Spitex.
The Dielsdorf Health Center plays a central role in the care provision of the region. As part of a pilot project to implement the new additional service regulation (ZLV), we are setting up a needs certification office.
Establishment and co-design of the needs certification office within the framework of the ZLV pilot project
Conducting needs assessments according to legal requirements (ZLV) for elderly people
Collection, analysis, and documentation of the individual living, care, and support situation
Consultation, support, and triage in complex cases (e.g., social or care crises)
Interdisciplinary collaboration with Spitex, care institutions, general practitioners, social services, and KESB
Ensuring quality and traceability of assessments (standardized documentation)
Active participation and evaluation of the pilot project until the end of 2027
Coordination with the contact and information point
Training as a nursing specialist HF/FH or equivalent qualification in health or social services
Further training and/or experience in geriatrics, case management, communication, or social counseling
Knowledge of regional service providers (care, Spitex, elderly work) is an advantage
Experience in outreach counseling (e.g., Spitex, social service, nursing homes)
Strong social skills, communication strength, and empathy
Enjoyment of development work and interdisciplinary collaboration
Independent, resilient, and proactive personality
