Job Description

• Assess the care needs of patients at home or hospitalized
• Define objectives and develop the care plan, in collaboration with the patient, their family, and the referring physician
• Implement and update personalized care plans according to the evolution of the patient’s health status by coordinating the interventions of the various health professionals in the multidisciplinary team
• Ensure the administrative follow-up of patient files and manage correspondence with network care actors
• Act as a liaison between patients, their families, and the multidisciplinary team
• Provide technical and emotional support to the care teams
• Initiate or support team sessions to optimize patient care and/or respond to the needs of one or more team members
• Plan staff and care
• Prepare quotes related to the service offer
• Be involved in the continuous improvement of practices
• Promote integration and supervise new collaborators and students

Qualifications

• Bachelor HES in nursing or equivalent training recognized by the Swiss Red Cross.
• Experience desired in home care and coordination
• Organization, adaptability, positive mindset
• Native French speaker or very good knowledge mandatory
• Foreign languages an asset
• Excellent computer skills
