Job Description
- Assess the care needs of patients at home or hospitalized
- Define objectives and develop the care plan, in collaboration with the patient, their family, and the referring physician
- Implement and update personalized care plans according to the evolution of the patient’s health status by coordinating the interventions of the various health professionals in the multidisciplinary team
- Ensure the administrative follow-up of patient files and manage correspondence with network care actors
- Act as a liaison between patients, their families, and the multidisciplinary team
- Provide technical and emotional support to the care teams
- Initiate or support team sessions to optimize patient care and/or respond to the needs of one or more team members
- Plan staff and care
- Prepare quotes related to the service offer
- Be involved in the continuous improvement of practices
- Promote integration and supervise new collaborators and students
Qualifications
- Bachelor HES in nursing or equivalent training recognized by the Swiss Red Cross.
- Experience desired in home care and coordination
- Organization, adaptability, positive mindset
- Native French speaker or very good knowledge mandatory
- Foreign languages an asset
- Excellent computer skills
