Our range of services includes both somatic and psychiatric care, counseling, household help and support, as well as the Spitex shop. In addition, we are a training and internship location and a member of the Gesundheitsmeile Uster training cooperation. With our help, clients can stay in their familiar environment or return home earlier after a hospital stay.

Spitex - Certified Nursing Specialist HF as Case Management Nursing 60%
Your main tasks
- Conducting needs assessment with InterRai HC
- Control, responsibility, and implementation of the entire nursing process
- Nursing and treatment of clients in their familiar home environment, even in complex situations
- Promotion of resources and implementation of health-preserving measures
- Interdisciplinary collaboration internally and externally (specialist offices, doctors, hospitals, and other services)
You bring
- Diploma as a Certified Nursing Specialist HF
- InterRai HC course or willingness to complete it
- High independence, good organizational skills, and resilience
- Communication skills and joy in active and supportive collaboration
- Optionally own car (with mileage reimbursement by Spitex) or driver's license category B

- Primarily regular working hours Monday to Friday, with the possibility of occasional home office. If you wish for a higher workload, it is possible to cover this through additional client assignments.
