Dipl. Nursing Specialist HF/FH (80-100%)
As a qualified nursing specialist in case management, you take on a central role with us and contribute significantly to the quality of life and satisfaction of our clients. Our managed teams are interdisciplinary, with high personal responsibility and plenty of room for creativity.
Your main tasks include:
- Taking over case management including needs assessment using InterRai HC, planning, implementation, and evaluation of nursing processes
- Ensuring high-quality, client-oriented care in the home environment
- Interprofessional collaboration with general practitioners, institutions, relatives, and other persons involved in the treatment
- Professional support and guidance of team colleagues in complex care situations
- Documentation and quality assurance according to applicable standards
You bring with you:
- Completed training as a Dipl. Nursing Specialist HF/FH
- At least 1 year of professional experience, preferably in Spitex
- Enjoyment of independent and responsible work
- Good IT user skills (digital nursing documentation)
- Either a category B driving license (no own vehicle required, but welcome) or the willingness to visit our clients by e-bike in all weather conditions
