What you will move with us
- You coordinate the rehabilitation process and manage the nursing process.
- Conducting anamnesis, family, location, and discharge interviews is also part of your responsibilities.
- You are responsible for advising patients regarding stay, discharge, aids, and follow-up solutions, in collaboration with the interdisciplinary team.
- You carry out needs assessments and discharge organization.
- In addition to the tasks as a Care Manager, you support the nursing team in the professional care and support of patients.
- Nursing clarifications with referrers are also part of your duties.

- With your completed training as a certified nurse AKP/DNII/HF and several years of professional experience (experience in rehabilitation nursing is an advantage).
- With your professional further education in case management, patient counseling, or your willingness to complete such training.
- Because you bring strong social skills and communication abilities as well as a high service mentality.
- With your ability to organize your work flexibly and being reliable and proactive.
