Tasks:
- Comprehensive support of our clients and their environment
- Interdisciplinary collaboration with other employees of livA spitex as well as relatives, specialists, and clinics
- Needs assessment, admission and discharge management
- Creating care plans and monitoring interRAI-CMH documentation
- Maintaining progress reports
- Creating prescriptions and updating them as needed
- Independent scheduling and organization with clients
- Active participation in thinking, shaping, and developing livA spitex
Requirements:
- Completed training as a Dipl. Nursing Specialist HF
- Experience in the psychiatric field
- Team player, open to constructive feedback, approachable
- Clear role understanding, high (self-)reflection ability, professional handling of closeness and distance
- Multilingualism and experience in the Spitex sector are advantageous
