Coordinator for the Lausanne Region Health Network
Context
The Lausanne Region Health Network is an association that brings together more than a hundred health institutions and professionals – CHUV, treatment and rehabilitation centers, nursing homes, home help and care (CMS), and doctors – as well as 61 municipalities.
With about a hundred collaborators, we contribute to patient orientation and continuity of care, notably through the Regional Information and Orientation Office (BRIO). We also carry out projects aimed at improving the quality and coordination of care, mainly benefiting an elderly population weakened by one or more chronic diseases.

Then you are the person we are looking for. Due to a retirement, we are seeking a COORDINATOR (80% to 100%)
Mission
- Anticipate and plan, in collaboration with other professionals, the orientation of the user within the care network. Based on your expertise (occupational therapy), assess functional needs and formulate appropriate orientation projects.
- Facilitate and coordinate continuity of care by communicating and acting as an interface/pivot between different partners, taking into account the user’s needs and the resources of the health system.
- Inform the user, their caregivers, and the professionals involved about options and their evolution, activate necessary services, and mobilize first-line actors.
Profile
- Higher education (Bachelor HES) required: we are actively looking for occupational therapists (as well as nurses) wishing to evolve towards coordination in community liaison.
- Professional experience in community or hospital settings and good knowledge of the Vaud socio-health network.
- Ability to work collaboratively with family, medical, and institutional networks, in an innovative context, with strong listening, empathy, and negotiation skills.
- Proficiency in common office applications and ease with administrative work.
- Analytical, synthesis, and creativity skills, as well as organizational and priority management abilities.
