Role Description
As part of the development of our activities, we are currently recruiting two social workers: one position in Lausanne and one position in Yverdon.
- Conduct social situation assessments, follow-up and social support for patients at our centre, taking into account the psychiatric dimension
- Work within a multidisciplinary team composed of psychiatrists, psychologists, nurses, neuropsychologists tasked with ensuring integrated psychiatric and psychotherapeutic follow-ups
- Coordinate the patient’s social care in collaboration with the various centre stakeholders and the network (CMS, CSR, SCTP, DGEJ, etc.)
- Conduct educational interventions and support in concrete processes (economic, administrative, professional, relational, legal, housing search assistance, finding a place in an institution)
- Assume the role of administrative referent, pending the establishment of an adult protection measure by the Justice of the Peace, and collaborate with the patient to avoid loss of social benefits
Your Profile
- Bachelor of Science in Social Work, social service orientation or equivalent qualification
- You have professional experience enabling you to work completely autonomously. Mental health experience required
- Competence in assessing the social situation of patients with mental disorders
- Experience in management and team leadership appreciated
