Context

The Lausanne Region Health Network is an association that brings together more than a hundred health institutions and professionals – CHUV, treatment and rehabilitation centres, nursing homes, home help and care (CMS) and doctors – as well as 61 municipalities.

With around a hundred employees, we contribute to patient orientation and continuity of care, notably through the Regional Information and Orientation Office (BRIO). We also lead projects aimed at improving the quality and coordination of care, mainly benefiting an elderly population weakened by one or more chronic illnesses.

Mission

• Anticipate and plan, in collaboration with other professionals, the orientation of the user within the care network. Based on your expertise (occupational therapy), assess functional needs and formulate appropriate orientation projects.
• Facilitate and coordinate continuity of care by communicating and acting as an interface/pivot between different partners, taking into account the user’s needs and the resources of the health system.
• Inform the user, their close caregivers and the professionals concerned about options and their development, activate necessary services and mobilise first-line actors.

Profile

• Commercial employee CFC or equivalent training
• Excellent contact and ease in relating to diverse audiences
• Strong interest in new technologies and digital projects
• Excellent command of common office tools
• Ability to work independently while appreciating teamwork
• Organisational skills, reliability and discretion
• Driving licence
