Your area of responsibility
- Consultation and support for patients and
their relatives in social matters - Referral to necessary social services
- Organization of the discharge of patients
- Promotion of cooperation with other social institutions, regional social services, and
nursing or disability homes
Your competencies
- Completion of training/studies in nursing at
tertiary level or comparable training/studies - Training in case management or willingness to complete it
- Preferably already professional experience in a similar institution
- Balanced personality with high
social competence, great willingness to serve, and the ability to maintain calm, overview, and solution orientation even in hectic situations - Independent working style, responsible, and high decision-making readiness
