Care manager within the Leenaards Memory Centre
As part of the CareMENS programme and to strengthen our passionate and dedicated team, we are looking for a Care Manager between 20% - 40%.
Context
The Lausanne University Hospital (CHUV) is one of the 5 Swiss university hospitals. Thanks to its collaboration with the Faculty of Biology and Medicine of the University of Lausanne and EPFL, CHUV plays a leading role in medical care, medical research and education.
The Department of Clinical Neurosciences brings together the services of neurosurgery, neurology, neuropsychology and neurorehabilitation, the Leenaards Memory Centre, the Neuroscience Research Centre as well as the Spinal Surgery Service. It offers integrated and personalised care to patients suffering from diseases of the central and peripheral nervous system, from brain to muscle.
Inaugurated in 2013 and led by Professor Gilles Allali, the Leenaards Memory Centre is a centre of excellence dedicated to the care and research of Alzheimer’s disease and other cognitive disorders related to ageing. Combining multidisciplinary clinical expertise and cutting-edge translational research, the CLM’s mission is to improve understanding, early diagnosis and management of neurodegenerative diseases.
Mission
You will be part of a dynamic team within the Leenaards Memory Centre (CHUV), which is the largest memory centre in Switzerland, with a multidisciplinary team composed of neuropsychologists, geriatricians, neurologists, psychiatrists, nurses and social workers.
You will work as a care manager within the CareMENS programme. Already well established at the CLM and in other memory centres in French-speaking Switzerland; CareMENS is a care model that offers management of patients with cognitive decline (Alzheimer’s disease and related syndromes) in physiotherapy, neuropsychology or speech therapy. To sustain the benefits of therapies, a community activity and support by a care manager are added.
You will take on this role of care manager, which is a pivotal role in the programme and within the Leenaards Memory Centre. CareMENS patient management first requires defining their needs, implementing the necessary intervention and support, ensuring follow-up and finally setting up a prevention plan, which is co-constructed with the patient.
In addition to being trained and supported in your role as care manager, you will have many other training opportunities in the field of neurodegenerative diseases, thanks to numerous internal and external conferences, seminars or colloquia covering a variety of theoretical, experimental and clinical topics.
Profile
- You have a HES nursing diploma (or equivalent qualification, as well as Red Cross recognition for staff qualified abroad, mandatory)
- You have specific knowledge in the field of neurodegenerative diseases
- You demonstrate rigour, organisation, interpersonal skills and ability to work in a multidisciplinary team
- You adopt an organised, meticulous, reliable and structured working method
- You are flexible
- You are able to work independently while being an effective team member
