The Aubonne Hospital has been serving its community since 1895. Ideally located near the vineyards and the village, it is one of the three founding hospitals of the EHC. It brings together all the missions of the EHC in one place: whether for primary care with its medical center, acute care with its recognized internal medicine beds and its palliative care unit, the multisite treatment and rehabilitation center, or long-term care services with its EMS and Temporary Reception Center (CAT).
We are looking for the Aubonne Hospital a:
Liaison Nurse (M/F) at 80%
To strengthen our Liaison service, starting January 1, 2026.

YOU HAVE
- A HES Bachelor in nursing or a nurse diploma recognized by the Swiss Red Cross.
- Training in community health or a CAS in care coordination and network work, an asset.
- Very good knowledge of the Vaud health system or a particular curiosity related to this function.
- Leadership, a true professional stance and strong skills in communication, negotiation and argumentation.
- A strong sense of listening and excellent interpersonal skills.
- A good overall vision of situations combined with an analytical mind allowing to establish links and consider all the issues of the orientation project.
YOUR OBJECTIVES
- Evaluate clinical situations in order to build the patient’s orientation after geriatric rehabilitation.
- Ensure liaison for the realization of the patient’s orientation project, taking into account their wishes and needs, in coordination with care network partners (relatives, doctors, hospitals, CMS, etc.)
- Collaborate with BRIO coordination nurses for orientation projects in EMS and with the interdisciplinary team to consolidate the orientation project.
- Organize and lead network meetings.
- Participate in interdisciplinary conferences.
