The public Spitex Wald Fischenthal is responsible for the outpatient healthcare of the residents of Wald and Fischenthal. The Spitex staff competently and empathetically care for their clients in their own homes, provide psychiatric support, or assist them in daily life. Spitex Wald Fischenthal is part of the non-profit Stiftung Drei Tannen, which, in addition to Spitex, operates the newly built Rosenthal nursing center and offers senior apartments with needs-based services.

We are immediately looking for an experienced Advanced Practice Nurse (APN) who takes case management responsibility for complex care situations and actively helps shape and further develop professional structures and forms of support within the team.

Advanced Practice Nurse (APN) & Deputy Head Spitex

Your area of responsibility:

• Case management as well as care and support of clients and their relatives with complex, chronic, and multimorbid care situations
• Conducting advanced nursing assessments as well as planning, implementing, and evaluating evidence-based nursing interventions
• Professional advice and coaching of nursing professionals in complex issues
• Participation in case discussions as well as in complex professional and ethical decision-making situations
• Reviewing, updating, and further developing nursing documentation
• Contributing to quality and development tasks
• Representing the nursing management in their absence

Your profile:

• Master of Science in Nursing (MScN)
• Several years of professional experience in case management in Spitex
• Confident and practical application of the InterRAI HC / CMH instruments
• Solid experience in caring for people with complex care situations
• High professional, advisory, and communication skills
• Independent, reflective, and responsible working style
• Enjoyment of interprofessional collaboration and active further development of leadership and APN roles
