To strengthen our newly created dementia team in Reinach, we are looking for you as
Certified Nursing Specialist HF or Health Specialist Dementia Department (60-90%)
What awaits you:
- Day shifts from Monday to Friday including public holidays
- Independent, competent and resource-oriented care of our clients and their relatives in dealing with dementia diseases in their familiar living environment
- Taking over case management for a defined client group
- Planning, control and monitoring of the care process
- Supporting our clients in a self-determined everyday life at home by promoting independence and quality of life
- Interdisciplinary cooperation with relatives, doctors and other services
- As a Certified Nursing Specialist HF, you are responsible for conducting assessments and status discussions with RAI-HC
What you bring:
- Completed training as a certified nursing specialist HF or completed vocational training as a health specialist EFZ
- Professional experience and broad expertise
- Additional training in the field of dementia is an advantage (or willingness to further educate in this area)
- An independent, responsible way of working
- High sensitivity and empathy
- Willingness to be on the road by car
