These are your tasks
- You conduct initial interviews with patients and relatives to plan discharge and coordinate the structured exchange of information with external service providers
- You ensure the complete and timely transmission of relevant documents to follow-up institutions and continuously document the discharge process in the clinical information system
- You work closely with doctors, nursing staff, social services and other specialist disciplines to create a smooth interprofessional process

- A diploma as a nursing specialist HF/FH and professional experience
- A working style characterised by high reliability as well as clear, appreciative and empathetic communication
- Strong organisational skills and the ability to maintain an overview even in complex situations
