Your tasks

• Participate in organising the patients' discharge project within a multidisciplinary approach
• Assess the psycho-social situation of patients and implement the necessary interventions
• Promote collaboration with the cantonal and extra-cantonal socio-health network

Your profile

• Bachelor of Arts HES-SO in Social Work with a focus on Social Services or an equivalent qualification
• Hospital experience and/or knowledge of the Neuchâtel health network is an advantage
• Good legal knowledge and social insurance
• Proficiency in common IT tools (MS Office)

Ease in interpersonal relations and communication skills

• Ability to work independently, in a multidisciplinary team and network
• Flexibility, good adaptability and responsiveness
• Resistance to stress and pressure
• Critical thinking, ability to analyse quickly and synthesise

• Workplace: Pourtalès site with mobility to other sites (Chaux de Fonds, Le Locle, and Val-de-Ruz)
• Start date: from 1 October 2026 or to be agreed, permanent contract
