About the job
Senseera Gesundheit AG will be a comprehensive provider in the field of long-term care and outpatient care outside hospitals in the Sense district from 01.01.2026. It unites eight nursing homes, a day care center, and an outpatient nursing service. As a reliable partner in healthcare, over 800 employees care for more than 400 residents daily and provide around 370 assignments for the clients of Spitex.
For our outpatient area (Spitex), we are looking at the Uttewil location (Bösingen), Team C immediately or by arrangement for a
Dipl. Nursing Professionals HF/FH 50% - 80%
What to expect
- Taking over case management for clients.
- Responsible for professional, client-related, continuous, and coordinated care and support
- Control and planning of the entire nursing process
- Conducting reassessments with the RAI HC instrument for the responsible clients to assess and register the care needs with the health insurance companies.
What you bring
- Degree as a certified nursing professional HF/FH and experience in outpatient care
- Advanced knowledge of RAI HC is an advantage
- Professionally competent, resilient, and flexible as well as enjoying independent work
- An open and communicative personality significantly contributes to good teamwork as well as interdisciplinary exchange with our partners in healthcare
- Handling PC and tablet is part of the daily work routine
- You have your own car available for assignments with our clients.
