About the job
What you can achieve
- Responsibility for carrying out discharge and transfer planning tailored to the care needs of patients, in collaboration with the treatment team
- Professional management and continuous optimisation of processes in discharge management as well as the further development of case management towards holistic support of patients throughout the entire treatment process
- Organising and coordinating customised follow-up solutions (outpatient and inpatient) taking into account the guiding principles of case management
- Advising and supporting patients, relatives and treatment teams in challenging patient situations within the responsible medical/nursing specialty
What you bring with you
- HF/FH nursing diploma, further training (CAS Case Management) or willingness to complete this promptly
- Several years of professional experience in the acute medical field
- Good IT user skills in common MS Office programmes
- Exceptionally high communication skills and service orientation
- Foreign language skills are an advantage
- High social competence and pronounced team orientation in a multiprofessional context
- Coordination and organisational skills combined with assertiveness and resilience
- Active support and implementation of change processes
