Spitex Zimmerberg AG is a non-profit organisation responsible for the care, advice, support and assistance of people of various age groups who rely on external help due to health limitations. We enable the residents of Adliswil, Thalwil, Kilchberg, Rüschlikon and Langnau am Albis to live a self-determined life in their familiar environment. Spitex Zimmerberg AG has around 160 employees spread over four locations and is thus one of the largest Spitex organisations in the canton of Zurich.

Your tasks
- Needs assessments according to interRAI-HC
- Independent, responsible care and support of clients
- Taking over case management
- Assessing care needs and care effort
- Creating, adjusting and evaluating care documentation according to interRAI-HC
- Carrying out and documenting interventions according to care planning
- Conducting counselling interviews
- Ongoing review of the quality and effectiveness of care
- Collaboration with relatives, doctors, hospitals, nursing homes and other specialist agencies
Requirements profile
- a completed education (HF/FH)
- professional, independent working style
- a broad professional experience
- social skills, resilience
- flexibility, especially regarding working hours
- good PC skills
- Driving licence category B
