About the job
The Réseau Santé la Côte (RSLC) is one of the four Vaud care networks according to the Cantonal Law on Care Networks (LRS). It brings together regional care providers (hospitals, nursing homes, community medical services, doctors, etc.) and municipalities. The network's mission is to improve, in close collaboration with its members, the coordination of care and the guidance of users within the healthcare system. The RSLC manages the Regional Information and Orientation Office (BRIO), whose main missions are: guiding people with loss of autonomy upon hospital discharge, managing and evaluating requests for nursing home placements, and providing information on medico-social services to the public and professionals.

A 50% BRIO Social Worker on a Permanent Contract
"Medical Practices"
Within the BRIO team, in collaboration with social workers dedicated to nursing home placements and hospital settings, you will offer your expertise to clarify social situations arising within medical practices. Your approach, experience, ability to build connections and refer cases to appropriate services enable vulnerable individuals to clarify their social situation and be supported throughout their care journey.
Your main activities, with the RSLC medical practices:
- At the doctor’s request, ensure the handling (initiating procedures and/or referral to other professionals) of social and insurance-related situations that need to be addressed for adult patients whose care benefits are compromised by social and/or financial difficulties (housing, lack of insurance, specific knowledge and/or support such as a curator).
- Be a resource person for social questions for the medical practices.
Your profile:
- Degree in social work (Bachelor of Arts HES-SO in social work) or equivalent training.
- CAS (in mental health and social psychiatry, interprofessional addictions, or social management) – an asset.
- At least 2 years’ experience in the healthcare sector (hospital, extra-hospital, and/or nursing home and/or medico-social institution), as well as network work experience.
- Very good knowledge of social insurance, the Vaud medico-social system, and procedures related to the Justice of the Peace.
- Strong writing skills and proficiency with common IT tools.
- Proven ability to work in a multidisciplinary team and network.
- Proactivity in the field of expertise.
- Negotiation skills, listening, and empathy.
- Vehicle and driving licence required.
