Your tasks with us are
- optimal care and advice in the field of palliative care
- case management responsibility and documentation of the nursing process with interRAI HC
- support for clients and relatives in difficult life situations
- collaboration with external agencies such as Palliativ Plus, hospitals, and general practitioners
- advice and guidance of nursing specialists in everyday life
What you bring with you
- completed training as a certified nurse HF/FH
- desirable: further training in the field of palliative care (CAS) and interRAI HC certificate
- empathetic, communicative personality with an independent working style
- driver's license or willingness to carry out assignments by e-bike
- IT skills in MS 365
