Your Tasks
Clinical APN Practice
Responsibility for the APN-led transition from hospital to home care
Early acquaintance with patients and relatives during the inpatient stay
Conducting home visits, clinical assessments, and progress evaluations
Initiation, evaluation, and adjustment of nursing and interprofessional measures
Promotion of self-management, education, and self-determination in chronic diseases
Team Leadership & Coordination
Professional and organizational leadership of the APN team
Coordination of deployments, case distribution, and prioritization of complex care situations
Ensuring a high-quality, evidence-based APN practice
Support, coaching, and professional development of team members
Active shaping of roles, processes, and interfaces in the interprofessional setting
Interprofessional & Cross-Sector Collaboration
Close cooperation with geriatricians, therapies, social services, and outpatient service providers
Strengthening cross-sector collaboration
Coordination of complex follow-up treatments and aftercare processes
Care Development & Research
Participation in care research, evaluation, and quality development
Contributing clinical expertise to project, concept, and further development work
Supporting the implementation of sustainable care models

Your Profile
Completed Master’s degree in Nursing (MScN) (or near completion)
Strong interest in Advanced Practice Nursing and integrated care in primary care
Ideally, initial experience in professional leadership, coordination, or project management
Pronounced clinical decision-making and analytical skills in complex situations
Very good communication and counseling skills
Leadership understanding with a cooperative, reflective leadership style
Independent, structured, and solution-oriented working method
