As an organization with six regional teams, an evening team, and the special services department, we competently and empathetically advise and care for people at home in their familiar environment in the Sense district of Fribourg, involving their relatives. In our catchment area with around 45,000 inhabitants, an interesting working environment awaits you.
For our Team A located in Flamatt, we are looking for someone to start immediately or by arrangement.
What awaits you
In the role of nursing specialist, you take over case management for clients. You are responsible for professional, client-centered, continuous, and coordinated care and support, as well as for planning and managing the entire care process. This also includes conducting reassessments with the RAI HC instrument for the responsible clients, to assess and register the care needs with the health insurance companies.
What you bring
You have completed training as a certified nursing specialist HF/FH and have a professional work attitude. You are professionally competent, resilient, and flexible and enjoy working independently. Your open and communicative personality significantly contributes to good teamwork as well as interdisciplinary exchange with our partners in healthcare. Working with PC and tablet is part of your daily routine. For assignments with our clients, you have your own car available.
