Your tasks include
- Receiving inquiries with initial situation assessment as well as situation and responsibility clarification
- Opening files/documentation of all interventions/progress reports
- Home visits and situation analysis
- Consultation on illness, support and relief options, social insurance, referral to other advisory institutions
- Roundtable discussion: organization and leadership, writing minutes
- Coordination conference: leadership and presentation of files and joint solution development
- Networking with partner organizations and external organizations
- Work on a geriatric ward at Zugersee Clinic as a certified nursing specialist HF
You bring with you
- Degree as certified nursing specialist HF / FH
- Experience in the field of counseling and/or dealing with the symptoms of cognitive impairment
- Very good German skills in spoken and written form
- High personal responsibility and proven social competence
- Enjoyment of teamwork
- Good networked thinking
