About the job
Your Tasks
- Organisation and coordination of rehabilitation for inpatient patients after hospital stay in close cooperation with case managers and social workers
- Ongoing exchange and coordination with medical staff, nursing service, rehabilitation clinics, cost bearers as well as patients and their relatives
- Continuous written documentation and prompt communication of transfer dates
- Triage of case registrations to social workers as well as acting as central contact person for external enquiries
- Assumption of various administrative tasks to relieve the management team and social workers
Your Profile
- Training / further education in healthcare
- Commercial further education
- Good MS Office skills
- Open personality with organisational talent and enjoyment of close teamwork
